Medial Displacement of Gastric Air Bubble on Conventional Radiograph

คราวนี้ขอแบบ old school diagnosis หน่อยนะครับ เริ่มต้นด้วย chest radiograph พร้อม label ให้ด้วยว่า air bubble ที่เห็นเป็น stomach. เห็น abnormality หรือไม่ครับ และมี differential diagnosis อะไรบ้าง?

ภาพด้านบนแสดงให้เห็นว่ามี abnormal soft tissue ที่ left upper quadrant ของ abdomen ซึ่ง displace gastric air bubble เข้าไปทาง medial. Soft tissue นี้อาจเป็น spleen (splenomegaly, splenic hemorrhage ในผู้ป่วย trauma), stomach (mass), massive ascites, etc.

รูปเฉลยอยู่ด้านล่างนี่เอง เป็น coronal reformatted CT แสดงให้เห็น splenomegaly ในผู้ป่วยอายุ 19 ปีที่เป็น Hodgkin's Lymphoma ครับ ข้อมูลเล็กๆ น้อยๆ แบบนี้อาจมีประโยชน์ถ้าเรารู้จักสังเกต และมองหา clue บนภาพๆ เดียว​​ เนื่องจากประวัติผู้ป่วยในใบ request อาจไม่บอกอะไรมากครับ

Differential Diagnosis of Splenomegaly

• Portal hypertension
• Infection (viral - mononucleosis, bacterial - TB/sepsis, rickettsia - typhus)
• Tumor (lymphoma, leukemia, myelofibrosis)
• Collagen vascular disease (SLE, RA)
• Storage disease
