Fat-Containing Lesion in Kidney

รูปด้านบน: contrast-enhanced axial CT แสดงให้เห็น mass ที่มีส่วนประกอบของ fat และ soft tissue (ส่วนมากเป็น fat) (yellow arrows) ที่ยื่นออกมาจาก upper pole ของ right kidney. ผู้ป่วยได้รับการทำ CT ด้วยสาเหตุอื่น และพบว่ามี renal AML โดยบังเอิญ.

Differential Diagnosis of Fat-Containing Lesion in Kidney

• Renal angiomyolipoma
• Renal lipoma
• Renal cell carcinoma engulfing perinephric fat โดยมากมักจะมี solid component เยอะกว่าตัวอย่างที่เห็น และก้อนมักจะมีขนาดใหญ่แล้วถึงได้ invade perinephric fat รอบๆ
  

Diagnosis: Renal angiomyolipoma, isolated

Facts about Renal AML

• Renal angiomyolipoma เป็น renal tumor ที่ประกอบไปด้วย fat, muscle และ blood vessels
• Renal AML อาจพบเป็น isolated disease (พบบ่อยกว่า, ดังตัวอย่างนี้) หรือร่วมกับกลุ่มโรค tuberous sclerosis และ sporadic lymphangioleiomyomatosis ก็ได้
• Complications ที่สำคัญมี 2 อย่าง คือ hemorrhage (Wunderlich syndrome) และ renal failure (จาก tumor ขนาดใหญ่ส่งผลให้ไตทำงานไม่ปกติ)
  
• Tumor ใหญ่กว่า 4 ซ.ม. ผู้ป่วยมักมีอาการและอาจต้องได้รับการผ่าตัด
• Angiomyolipoma อาจมีขนาดใหญ่ขึ้นได้เมื่อ follow up
