Tooth Aspiration Following Facial Trauma


ภาพ chest x-ray แสดงให้เห็นว่ามี abnormal opacity อยู่บริเวณ right hilum. ไม่พบว่ามีลักษณะของ air trapping หรือ lung opacity ในบริเวณของ right middle หรือ right lower lobe.


ภาพ CT scan (coronal reformat) แสดงให้เห็น foreign body ที่มี density เข้าได้กับ calcium, ร่วมกับรูปร่าง และ facial CT ที่พบว่ามี missing tooth, ลักษณะดังกล่าวเข้าได้กับ tooth aspiration ใน right lower lobe bronchus.

Tooth Aspiration Following Facial Trauma:

  • มีองค์ประกอบสำคัญ 3 อย่าง คึือ facial trauma, loss of gag reflex (ผู้บาดเจ็บไม่รู้สึกตัว) และ missing tooth (either seen clinically or by imaging)
  • ต้องทำ chest และ abdomen x-ray ในผู้บาดเจ็บดังที่กล่าว เพื่อดูว่ามี tooth aspiration เข้าไปใน airway หรือ GI tract
  • ส่วนใหญ่ aspiration เข้าไปใน right bronchus

5 Simple Steps to Improve Pediatric Radiological Care Concerning Radiation Safety


The following is a direct quote from ImageGently.org, a newly organized international alliance for radiation safety in Pediatric imaging.


Here are 5 simple steps to improve patient care in your everyday practice:

  1. Increase awareness for the need to decrease radiation dose to children during CT scanning. Protocol development recommendations are offered in ImageGently.org
  2. Be committed to make a change in your daily practice by working as a team with your technologists, physicist, referring doctors and parents to decrease the radiation dose! Sign the pledge! Click on the link on the home page to join the image gently campaign today.
  3. Contact your physicist to review your adult CT protocols and then use the simple CT protocols on this website to “down-size” the protocols for kids. More is not better….adult size KV and mAs are not necessary for small bodies.
  4. Single phase scans are usually adequate. Pre- and post contrast, and delayed CT scans rarely add additional information in children yet can double or triple the dose! Consider removing multi-phase scans from your daily protocols.
  5. Scan only the indicated area. If a patient has a possible dermoid on ultrasound, there is rarely need to scan the entire abdomen and pelvis. “Child-size” the scan and only scan the area required to obtain the necessary information.

Epiploic Appendagitis

Epiploic appendage เป็น fat ที่เห็นเป็นริ้วๆ เกาะอยู่ตามขอบของ colon ซึ่งอาจเกิดการอักเสบขึ้นได้เนื่องจากการบิดตัว ที่นำไปสู่ vascular obstruction และ ischemia. ผู้ป่วยมาด้วยอาการปวดท้อง ซึ่งมักเป็นการปวดท้องช่วงล่างด้านซ้าย (บ่อยกว่าด้านขวา). อาการปวดอาจคล้ายกับ acute diverticulitis หรือ acute appendicitis. ใน CT scan จะเห้นว่ามี inflammation ใกล้กับ colon (เห็นเป็น fat density lesion ล้อมรอบด้วย soft tissue stranding)(red arrow) และจะเห็นว่า colon ใกล้เคียง (blue arrowheads) ไม่มี wall thickening ซึ่งเป็นสิ่งสำคัญที่ช่วยบอกว่าลักษณะที่เห็นไม่ใช่ acute diverticulitis.

Epiploic appendagitis is an inflammation of the colonic epiploic appendages. It is believed to be due to torsion of the appendages, resulting in vascular occlusion and ischemia. Clinically, it mimics acute diverticulitis, or less commonly acute appendicitis. The key imaging features to differentiate between acute epiploic appendagitis with diverticulitis are central fat density and absence of adjacent colonic wall thickening.

ASD with PAPVR

รูป 1 - Chest x-ray ของผู้ป่วยหญิงอายุ 20 ปี แสดงให้เห็นลักษณะของ increased pulmonary vasculature (shunt vascularity) ซึ่งพบได้ใน ASD, VSD, PDA หรือ PAPVR

รูป 2 - Axial CT แสดงให้เห็น Right upper lobe pulmonary vein (yellow arrow) draining into SVC, เข้าได้กับ Partial Anomalous Pulmonary Venous Return (PAPVR).

Atrial septal defect (ASD)

  • พบประมาณ 30% ของ congenital heart disease ที่วินิจฉัยในผู้ใหญ่
  • ส่วนมากเกิดจาก spontaneous genetic mutation
  • มี 4 ชนิด คือ ostium secondum (most common), ostium primum, sinus venosus, coronary sinus (least common)
  • Pathophysiology ขึ้นกับขนาดของ ASD, ถ้าขนาดใหญ่อาจทำให้เกิด left to right shunt, pulmonary hypertension, right ventricular failure.
  • CXR: prominent pulmonary vasculature, RA and RV enlargement, PA dilatation (ในคนไข้ที่มี hemodynamically significant shunting)
  • วินิจฉัยได้จาก echocardiography โดย secondum and primum defects เห็นได้โดย transthoracic echocardiography. ส่วน sinus venosus defect เห็นได้ดีกว่าด้วย transesophageal technique.
ข้อควรรู้
  • Sinus venosus ASD มักพบที่ junction ของ superior vena cava
  • Sinus venosus ASD มักพบร่วมกับ PAPVR

Pylephlebitis

  • Also known as: portal or mesenteric vein thrombosis, ascending septic thrombophlebitis.
  • Acute ascending infection of portal vein and its tributaries often arising from primary GI inflammatory lesion (common: diverticulitis, appendicitis, infected pancreatic necrosis).
  • High mortality rate of more than 50%.
  • Radiologic findings: primary source of infection (appendicitis, diverticulitis, etc), intravascular thrombi or air in the portal veins or mesenteric veins.

Pylephlebitis เป็นการอักเสบของ portal vein และ/หรือ mesenteric veins ซึ่งเกิดจาก infection/inflammation ของ organ ที่ supply ด้วย portal vein/mesenteric veins (ที่พบบ่อยเกิดจาก acute diverticulitis, acute appendicitis, pancreatic necrosis). การตรวจพบอย่างถูกต้องและแม่นยำมีความสำคัญเนื่องจากอัตราเสียชีวิตสูง. Imaging สามารถแสดงให้เห็น source of infection (CT อาจเป็นวิธีที่เหมาะสมเนื่องจากสามารถวินิจฉัย diverticulitis และ appendicitis ได้ดี) และ thrombus ภายใน veins ได้ (CT, ultrasound, MRI). สำหรับเคสตัวอย่าง ไม่พบ source of infection แต่ผู้ป่วยมีประวัติ CMV colitis (confirmed by pathology) มาก่อนแสดงอาการในครั้งนี้ ทำให้ได้รับการวินิจฉัยว่าเป็น septic thrombophlebitis (pylephlebitis).
