How Doctors Think: Chapter 2

ในบทนี้ชื่อว่า Lessons from the Heart, Dr. Groopman เล่าเรื่องของ Evan ซึ่งเป็น park ranger อายุประมาณ 40 ปี สุขภาพแข็งแรง และฟิต มีอาการเจ็บหน้าอกฉับพลันขณะกำลัง jog อยู่ใน park จึงไปตรวจที่ ER. แพทย์คนตรวจซักถามประวัติอย่างละเอียด ส่ง EKG และ cardiac enzymes ซึ่งผลปรากฎว่า normal จึงส่ง Evan กลับไป. วันถัดมาหมออีกคนมาบอกว่า Evan มาตรวจใหม่วันเดียวกันและได้รับการวินิจฉัยว่าเป็น acute myocardial infarction. บทเรียนที่ Groopman สอนเป็นเรื่องของ negative stereotype เนื่องจาก Evan เป็นคนไข้ที่แข็งแรง ออกกำลังกายสม่ำเสมอและสภาพร่างกายฟิต ทำให้หมอประเมินว่าไม่น่าจะเป็นอะไร ทั้งที่จริงๆ Evan มีอาการของ unstable angina ตอนที่มาตรวจในครั้งแรก.

อีกตัวอย่างซึ่งคล้ายกับเคสของ Evan คือ Brad ซึ่งเป็นนักวิ่ง ป่วยเป็น osteosarcoma และเข้ารับการให้เคมีบำบัดก่อนทำการผ่าตัด. Brad มีไข้สูงระหว่างการให้เคมีบำบัด แต่ Groopman ตรวจร่างกายไม่ complete โดยเลือกที่จะละการตรวจ rectal exam ไป (เนื่องจากความสงสารว่า Brad ทรมานจากการให้เคมีบำบัด) ทำให้ miss perianal abscess ซึ่งเป็นสาเหตุของไข้และนำไปสู่ septic shock. โชคดีที่ Brad สู้กับ infection ได้และผ่านขบวนการไปสู่่การผ่าตัด

ในอีกแง่มุมคือเรื่องของ Negative stereotypes เช่นผู้ป่วยที่ดูสกปรก มอมแมม สูบบุหรี่ ดื่มเหล้า พวกหมอมักจะคิดว่าไม่ดูแลตัวเอง ทำให้หมอต้องเสียเวลามารักษาโรคที่น่าจะป้องกันได้. ตัวอย่างน่าสนใจที่ Groopman กล่าวถึงเป็นผู้ป่วย diabetic coma ซึ่ง miss diagnosis ว่าเป็น drunken homeless (ผู้ป่วยจริงๆ แล้วเป็นนักศึกษา college ที่พบว่า coma อยู่หน้าพิพิธภัณฑ์, กลิ่นเหล้าจริงๆ แล้วเป็นจาก diabetic coma).

Groopman ยังยกตัวอย่างของ Ellen ซึ่งเป็นผู้ป่วย menopause ที่มีอาการร้อนรุ่มไปทั่วตัว มีอะไรมาไต่ตัว ปวดหัว ซึ่งคล้ายกับว่าเป็นอาการของ menopause. Ellen ยืนยันว่าตัวเองเป็นคน "แปลก" ก็จริง แต่อาการที่มีนั้นมากกว่า menopause ปกติ. จริงๆ แล้ว Ellen ป่วยเป็น pheochromocytoma.

In this chapter, Groopman shows us many cases of positive and negative stereotyping that develops in doctors' minds. Positive stereotyping is difficult so see and quite dangerous. It is a good feeling toward a certain group of patients who are supposely healthy, although they may not be. This can lead to delayed or missed diagnosis. Also, positive feeling toward patients as friends may lead doctors to delay or dismiss important and vital information gathering (such as lab test, physical exam in certain parts of the body, or invasive investigations). Negative feeling is easier to detect. This includes negative feeling of doctors toward patients with alcoholic cirrhosis, emphysema due to smoking, etc.


*Jerome Groopman ปัจจุบันเป็น Professor (medicine) ที่ Beth Israel Deaconess Hospital (Boston). เขาจบแพทย์จาก Harvard Medical School และทำ internship กับ medicine ในรั้ว Harvard ที่ Massachusetts General Hospital. เขาเคยทำงานที่ UCLA Medical Center ก่อนจะย้ายกลับมาอยู่ที่บอสตัน.
