Three 'Reasonable and Appropriate' Indications for Coronary CTA for Detection of CAD

Indications ที่เหมาะสมในการทำ Coronary CTA for detection of CAD มี 3 ข้อ

1. Symptomatic patients with intermediate pretest probability who have either uninterpretable EKG or unremarkable EKG
2. Symptomatic patients with uninterpretable stress test
3. Symptomatic patients with new onset of heart failure

จะเห็นว่า coronary CTA ไม่เหมาะสมที่จะทำในผู้ที่ไม่มีอาการ เนื่องจากอาจนำไปสู่ intervention ที่ไม่จำเป็นและเพิ่มความเสี่ยงอื่นๆ ต่อผู้ป่วยได้ และไม่เหมาะในผู้ป่วยที่เป็น high risk เนื่องจากการทำ cardiac catheterization เหมาะสมมากกว่า (สามารถทำ intervention ได้, gold standard)

สรุปว่า
Coronary CTA is appropriate in SYMPTOMATIC patients with INTERMEDIATE pretest probability
